The Integrated Independence Team (IIT) is an integrated health and social care service provided jointly by the Homerton University Hospital Foundation Trust and the London Borough of Hackney.
The service places the user at the heart of the care planning process and aims to promote independence, deliver outcome-focused care, and provide a seamless journey through the care system.
To manage a caseload of individuals and carers who require additional support or guidance in order to identify their needs and who may need close monitoring or frequent reviews and interventions to minimise and manage risk or where best interest decisions may be required.
To provide assistance, guidance and direction to colleagues and assist senior colleagues and managers as required.
To review service users receiving reablement care and assess as to whether long term support under the Care Act 2014 is required.
