As a Consultant Social Worker within Adult Services, you will be key to providing support to Practitioner’s at all levels of their career development and utilising your social work expertise around the development of the Hackney Social Work Practice Academy.
You will have a sound knowledge of the key principles of the Care Act and all other relevant legislation relating to the provision of services for adults with long term conditions. You will have a commitment to social work education for all and be ready to develop our Social Work offer through the academy approach.
A commitment to working in an integrated manner with health colleagues is essential, as well as an understanding of the value of working with the wider multidisciplinary team such as the Police, Housing, Education and Children’s services.
We are looking for someone who can make a positive contribution to the work of Adult Services as a whole , and by offering the provision of professional and reflective supervision, advice, support, as well as mentoring and co work with team members by contributing to more complex assessments and support planning. You will be an enthusiastic proponent of promoting every individual service users independence wherever possible, with a sound understanding of personalisation including use of Direct Payments.
You will need to be Social Work qualified or equivalent and registered with HCPC with substantial post qualification experience within statutory adult services.
We are currently recruiting two posts, one within our Integrated Hospital Discharge Service and one within our Information and Assessment Team.
