The North and South Hackney Recovery Teams provide an assertive and responsive service to adults aged 18 – 65 years who experience severe and enduring mental health illness within the borough of Hackney. We respond to the needs of adults with a mental disorder and their carers in a timely manner, within the boundaries of statutory legislation, and with regard to individual circumstances.
We assess need using a person centred and recovery focus approach, in conjunction with effectively assessing and managing risk. We plan and deliver outcome focused interventions linked to promoting independence, social inclusion and recovery.
The North and South Hackney Recovery Teams supports people to build resilience and restore quality of life using a bio-psycho-social model. The MDT team works collaboratively with service users and their carers/supporters to continually improve the service.
Your enthusiasm for providing a high quality service will be matched by your ability to work collaboratively with service users who are experiencing a range of mental health and social care needs, as well as providing support and assessment to their carers and families.
An excellent understanding of the issues people with mental health needs may experience is essential for this post as is a good understanding of Care Programme Approach and Case Management.
