Hackney Council is looking for a Senior Practitioner Social Worker to join TheCommunity Mental Health Care for Older People Team and Diagnostic memory clinic.
This team provides person and family-centred care for older people aged 65 yearsand over with complex and severe functional mental illness (including schizophrenia,Bipolar affective disorder, Emotional Unstable Personality disorder, depression, anxiety) and organic illness – Dementia living in City and Hackney area of London.
This is an exciting opportunity as it is a new role within the team. Together with the Operational Lead you will manage and oversee the Community Mental Health Carefor Older People Team and provide high quality and effective assessments ofindividuals with mental health problems and cognitive impairment referred to theservice.
This role requires extensive experience within a mental health setting combined withexperience of working as part of a multi-disciplinary team. Experience of making fullpsycho-social assessments and writing reports is also essential for this role. You willbe Social Work England registered and have a CQSW/DIPSW and AMHP qualificationor equivalent. Evidence of continual professional development is also key to thisrole.
