About the Safeguarding and Dols Team
The team consists of a Team Manager, 3 senior practitioners, Dols lead, social workers, information officers and business support staff.
About the Role
As a Senior Practitioner in the Adult Safeguarding Team, you will lead on complex safeguarding enquiries under the Care Act 2014, supporting adults at risk of abuse or neglect while promoting choice, control and empowerment. You will manage a complex caseload, provide professional leadership within the team, and work closely with partner agencies to achieve positive and proportionate outcomes.
You will also play a key role in supporting the professional development of colleagues through mentoring, reflective discussion and modelling high-quality practice.
The post of Senior Practitioner (SafeguardingTeam)is fundamental to embedding strength-based practice and making safeguarding personal.
Successful candidate will be supporting Team Managers in the following areas:
- Providing line management supervision to social workers
- Responsible for Safeguarding Manager duties (SAM)
- Quality Assuring Care Act Assessments, Mental Capacity Act assessments and Court of Protection applications and Human Rights Assessment
- Coordinate and contribute to multi-agency safeguarding meetings, including strategy discussions and case conferences
- Leading reflective practice and in-house training events.
- You will have a commitment to your own and your supervisees social work education and be ready to help and develop our Social Work offer through the academy approach.
- Work collaboratively with adults, carers, advocates and partner agencies to reduce risk and promote wellbeing
About You
You will be an experienced and confident practitioner with a strong commitment to adult safeguarding and rights-based practice.
Qualifications and Professional Registration
You will need to be a qualified Social Worker and be registered with Social Work England.
You will also have a significant level of post qualification experience within statutory Adult Services including providing supervisory / management support to social work practitioners
Experience of working within a Safeguarding team or acting as a SAM is desirable
An Enhanced DBS is required for this role
The post of Senior Practitioner (Continuing Health Care Lead)
We are seeking a highly motivated and experienced social care professional to join our ‘Move on team’ (Adult Social Care) as a Senior Practitioner and CHC Lead. This is a rewarding opportunity to make a significant impact on patient care pathways, ensuring individuals with complex health needs receive the fully funded care they are entitled to under the National Framework for NHS Continuing Healthcare and NHS Funded Nursing Care.
About you:
You will provide professional leadership, expert advice, and supervision to a team of practitioners, ensuring the delivery of high-quality, legally compliant, and person-centred assessments and reviews.
You will act as the nominated lead for a defined, complex caseload of patients whose care is funded by the organisation.
You will coordinate and undertake comprehensive, person-centred assessments for CHC eligibility in line with the National Framework.
You will provide leadership, support, supervision, and line management to a range of practitioners, driving continuous service improvement and best practices.
You will work collaboratively with key partners, patients, and their families, representing the organisation at various meetings and engaging with statutory and non-statutory bodies.
Qualifications and Professional Registration
- You will need to be a qualified Social Worker and be registered with Social Work England.
- You will also have a significant level of post qualification experience within statutory Adult Services and a high level of understanding of the NHS continuing healthcare framework
- You will have experience of providing supervisory / management support to social work practitioners.
