We currently have a number of positions in our Complex Care Service within Adult Social Care for team managers, senior practitioners and reviewing officers. This service covers case management and review of Hackney residents.
Senior Practitioners (Case Management and Review)
We are seeking practitioners who have a sound knowledge of the key principles of the Care Act, the Mental Capacity Act and Safeguarding, as well as all other relevant legislation relating to the provision of services for adults.
You will also have a significant level of post-qualification experience within statutory Adult Services, including providing supervisory / management support to social work practitioners.
Review Team
The Review Team is made up of reviewing officers and, with senior social worker support, helps us to ensure people remain safe and well cared for. The team deals with lots of complexities and utilises their skills in social care and with colleagues to help ensure care and support plans meet needs and enable stability for the individual. We are really proud of the number of reviewing officers who have accessed or are on the Social Work Apprenticeship.
An enhanced DBS check is required for all roles.
